American Diabetes Association is the nation’s leading 501(C)3 nonprofit health
organization providing diabetes research, information and advocacy. Founded in
1940, the American Diabetes Association conducts programs in all 50 states and
the District of Columbia,
reaching hundreds of communities.
The
mission of the Association is to prevent and cure diabetes and to improve the
lives of all people affected by diabetes.

fulfill this mission, the American Diabetes Association funds research,
publishes scientific findings, provides information and other services to
people with diabetes, their families, health professionals and the public. The

America
On the Move (AOM) is an obesity prevention program that began in 2001 as
Colorado On the Move (COM) under the direction of Dr. James O. Hill, Director,
Center for Human Nutrition. AOM is supported by the Friends of the Center for
Human Nutrition at the University of Colorado at Denver.
As a result of the success in Colorado,
AOM was launched as a national program in 2003. Currently there are AOM
programs and tools for schools, worksites, families, and healthcare
professionals.

The Colorado Physical Activity and Nutrition (COPAN) Program’s mission is to preventobesity and related chronic diseases, and to promote healthy lifestyles for all Coloradans.One of COPAN’s main charges is to implement the Colorado Physical Activity andNutrition State Plan 2010. The plan addresses education, community outreach, policy,and environmental support in schools, worksites, healthcare settings, and communities. The COPAN logic model or program plan, shows how COPAN’s resources and activities
are designed to change knowledge, attitudes, behavior, and policies around obesity, physical activity, and nutrition. These changes lead to the goal of reduced obesity-related chronic diseases such as cardiovascular disease, cancer, diabetes, and hypertension.
The Colorado Diabetes Prevention and Control Program mission it to
build capacity through statewide partners by providing guidance in
leadership and resource development and to promote and facilitate the
exchange of diabetes data and information to assist in program planning,
implementation, and evaluation. The DPCP supports statewide efforts to
prevent diabetes and its complications through building and maintaining
relationships with community partners and stakeholders.

Kaiser Permanente is the
largest private health care provider in Colorado with more than 475,000
members in the Denver, Boulder and Colorado Springs areas. Kaiser
Permanente owns and operates 17 medical offices, four mental health and
chemical dependency offices and state-of-the-art health care
technologies, such as its electronic medical record system, KP
HealthConnect®. Kaiser Permanente physicians and care teams focus on
prevention as well as managing and treating illness, all in an effort
to help patients live well and thrive, thus avoiding the burdens and
costs of chronic conditions to individuals and the health care system.
As a nonprofit organization, Kaiser Permanente is committed to
improving the health of communities. It partners with key organizations
to tackle challenges such as decreasing the number of uninsured,
improving health care affordability and public health issues such as
obesity. Kaiser Permanente has recently awarded LiveWell Colorado a $16
million grant over a five-year period. LiveWell Colorado will be one of
the first of its kind in the U.S. to serve as a central hub for
overseeting and optimizing healthy eating and active living initiatives
throughout the state.
